In order to assist us to make a full assessment of the potential risks to your health from your planned travel, we would ask that you make a Travel Clinic appointment with our Practice Nurse. Ideally, this appointment should be made around the time of booking your holiday, at least 6 weeks before you travel. Unfortunately, if you are travelling in less than two weeks time, we may be unable to offer you a travel appointment, and would ask that you attend a private travel clinic. We can provide details of local clinics if you wish.
Travel risks vary considerably, and for this reason we allow a minimum of 20 minutes for each appointment. We would aim to give you any necessary vaccinations at this appointment, to save you having to return. As some vaccines are not available on the NHS there may be a private fee charged and this will be discussed at your appointment. Please bring cheque book or cash with you to allow for this. It would help us if you could download and complete the following questionnaire and return it to the surgery at least 7 days before your appointment. Failure to return the completed questionnaire in time, means the travel nurse will be unable to see you and you will be asked to re-book.

If a patient does not attend their travel appointment they will be asked to contact a private clinic unless there is a good reason why they didn't come. We must also have plenty of time to re-arrange the appointment.
